What information will I need to register the Death

You will need to bring a Medical Certificate Cause of Death. This is normally issued to relatives by a doctor who had treated the deceased within 28 days of the date of death. It is not necessary to bring the deceased’s Medical Card.

If the deceased was not been seen by a doctor within 28 days of their death or if the death was not caused by natural illness, the case may be referred to the Coroner.  In this instance the staff at the Coroner’s Office will advise when it may be possible to register the death.

When you attend the office to register you will have to fill in a death registration form.

  • It is possible to download a Death Registration Form with explanatory notes at www.nidirect.gov.uk. The details you fill into this form are needed to complete the Death Registration. The Funeral Director may give you this form as it is helpful to the family to have some time to complete at home prior to visiting the office.  However if you wish you can complete a form when you visit the office.

What will I receive?

Once you have registered the death, you will be given the following documents:

  • Form 36 which can be given to Social Security offices regarding benefits.
  • GRO21 where a funeral has not taken place or the GRO21 is required for cremation purposes or removal of the remains outside the jurisdiction.

Certified copies of the death certificate, which may be needed to deal with business matters and insurance claims, may be purchased from the Registrar at the time of registration.

There is a charge of £8 per copy for this service.
